How to Read Your Anthem Insurance Card
When you verify Anthem benefits with our admissions team, we'll ask for a few numbers printed on your card. Here's exactly where to find them:
- Member ID / Subscriber ID — usually printed on the front of the card, top-right. Sometimes labeled "ID #" or "Enrollee ID."
- Group Number — front of the card, often below the member ID. Employer-sponsored plans always have one.
- Plan Type — look for "PPO," "HMO," "EPO," or "POS" printed on the front. Determines referral requirements.

What If Anthem Denies Your Rehab Claim?
