    What If UMR Denies Your Rehab Claim?

    Does UMR cover addiction treatment in New Jersey?

    Yes, in nearly all cases. UMR administers behavioral health benefits for self-funded employer plans, and parity rules mean PHP, IOP, MAT and detox are covered when medically necessary. The exact cost-sharing is set by your employer's plan document.

    Is UMR the same as UnitedHealthcare?

    UMR is a UnitedHealthcare company that administers claims for employers who fund their own plans. You often get access to the United provider network, but the benefit design and the money behind the claims come from your employer.

    Who manages behavioral health on a UMR plan?

    Frequently Optum, United's behavioral health arm. Check the back of your card for a separate mental health or substance use phone number. If it routes to Optum, authorizations go through Optum rather than UMR directly.

    Does UMR require prior authorization for PHP and IOP?
