What Addictions Does UMR Cover Treatment For?
Under the Mental Health Parity and Addiction Equity Act (MHPAEA) and the Affordable Care Act, UMR is required to cover substance use disorder (SUD) treatment at the same level as medical and surgical care. How to Read Your UMR Insurance Card
When you verify UMR benefits with our admissions team, we'll ask for a few numbers printed on your card. Here's exactly where to find them:
- Member ID / Subscriber ID — usually printed on the front of the card, top-right. Sometimes labeled "ID #" or "Enrollee ID."
- Group Number — front of the card, often below the member ID. Employer-sponsored plans always have one.
- Plan Type — look for "PPO," "HMO," "EPO," or "POS" printed on the front. Determines referral requirements.
- Behavioral Health / Mental Health Phone — on the back of the card. This is the number we call to verify your addiction treatment benefits.
- RxBIN & RxGroup — back of the card. Used to verify pharmacy coverage for Suboxone, Vivitrol and other MAT medications.

What If UMR Denies Your Rehab Claim?
UMR denials happen — usually over "medical necessity," missing prior authorization, or a documentation gap. A denial is not the end of the road. Federal parity law gives you the right to appeal, and Cherry Hill Recovery Center handles the entire process on your behalf at no cost.
- 1. Peer-to-peer review. Our medical director speaks directly with the UMR reviewing physician to justify medical necessity. Most denials reverse at this stage.
- 2. Formal internal appeal. If the peer-to-peer doesn't resolve it, we submit a written appeal with clinical documentation, ASAM criteria and prior treatment history — typically within 5 business days.
- 3. External / independent review. If UMR upholds the denial, NJ law entitles you to an independent third-party review through the NJ Department of Banking & Insurance (IHCAP program).
- 4. State parity complaint. If your denial appears to violate MHPAEA (mental-health parity), we help you file with the NJ Attorney General's parity enforcement unit.

Does UMR cover addiction treatment in New Jersey?
Yes, in nearly all cases. UMR administers behavioral health benefits for self-funded employer plans, and parity rules mean PHP, IOP, MAT and detox are covered when medically necessary. The exact cost-sharing is set by your employer's plan document.
Is UMR the same as UnitedHealthcare?
UMR is a UnitedHealthcare company that administers claims for employers who fund their own plans. You often get access to the United provider network, but the benefit design and the money behind the claims come from your employer.
Who manages behavioral health on a UMR plan?
Frequently Optum, United's behavioral health arm. Check the back of your card for a separate mental health or substance use phone number. If it routes to Optum, authorizations go through Optum rather than UMR directly.
Does UMR require prior authorization for PHP and IOP?
Usually yes, using ASAM medical-necessity criteria. Our clinical team submits the authorization and manages continued-stay reviews so you are not on the phone chasing it.
Does UMR cover Suboxone, Sublocade or Vivitrol?

Does UMR cover virtual IOP?
Most UMR plans reimburse behavioral telehealth at parity, so our Virtual IOP is generally available to UMR members statewide. We confirm during verification.
I have UMR through a union. Does that change anything?
